Received your results and need a HSA/FSA receipt? Or curious about if you can submit your purchase to your personal insurance? Read on!

Great question! Currently Fellow tests are self-pay, and we are not able to provide any assurance regarding insurance coverage or reimbursement. FSA/HSA plans are commonly used to pay for the test; however, we suggest confirming eligibility with the plan administrator first to be sure. If you are seeking insurance reimbursement, a Itemized Lab Services Receipt may be used (see below for more information on this).


Requesting a HSA/FSA Receipt

To ensure we are able to provide you with optimal (and speedy!) support, we now have a way for you to submit tickets directly to our Customer Success team.

Once a ticket is submitted, a member of our team will take a deeper look into your support request and follow up with the best next steps to resolve. To start your ticket request, please follow the steps below! πŸ‘‡

Submitting a Ticket:

  1. Log into your Fellow portal and click on the golden chat bubble located at the bottom right-hand corner of your screen.

    1. We may ask you to provide some information at first, we can best address your support request.

  2. Select an option from the Ticket Menu, that best matches your request.

    1. For help with your order, you'll likely want to select "Order Support"!

  3. Fill out and submit your ticket.

  4. You're done!

After we've reviewed your ticket, a member of our Customer Success team will be in touch within 2 business days, if not earlier. You can find updates on the status of your ticket in the chat directly, and these will also be shared with you automatically via email!


Insurance Reimbursement

Fellow is not able to provide any assurance regarding insurance coverage or reimbursement; however, an Itemized Lab Services Receipt may be used. Here's how to access the correct receipt for your kit type and steps you'll need to take before submitting -

  1. There are several versions of the Itemized lab Services Receipt, so you'll need to select your specific kit type from the options linked below. This will provide you with the proper receipt.

    1. Semen Analysis kit Receipt - Patient+Fillable+FSA+Receipt.pdf

    2. Semen Analysis + Cryopreservation Receipt - Patient+Fillable+CRYO+Receipt.pdf

    3. Vasectomy kit Receipt - Patient+Fillable+PVSA+Receipt.pdf

  2. Once you've downloaded the correct receipt, you'll need to fill it out with the following information, before submitting.

    1. Locate the Patient Information section and fill out using the sample producer's full name, DOB, and address.

    2. Locate the Service Provided area, and find Date of Service line item. The Date of Service is the same day you received your results from Fellow.
