Insurance and HSA/FSA receipts

Received your results and need a HSA/FSA receipt? Or curious about if you can submit your purchase to your personal insurance? Read on!

Fellow is an in-network semen analysis Provider for eligible Progyny and Maven members. While we cannot guarantee insurance coverage or reimbursement, we would be happy to provide you with a detailed receipt of your semen analysis test purchase for insurance or HSA/FSA reimbursement consideration.

Although FSA/HSA plans are commonly used to pay for our tests, we suggest confirming eligibility with your plan administrator before making your purchase to be sure that it is an approved expense. If you are seeking insurance reimbursement, a Itemized Lab Services Receipt may be used.

It is important to note that this receipt can only be generated after your results have been released to you!


Obtaining Your Itemized Receipt

  • Log in to your Fellow portal at you.meetfellow.com

  • In the top right corner of your screen, open the dropdown menu by clicking on your name and then click Receipts

  • You should now see a disclaimer regarding receipt submission to government payers

  • Once you click “I agree” and Next, you will see a screen with a purple PDF link where you can access your itemized receipt. If you have purchased multiple kits, you will see multiple links here. The unique specimen container barcode ID for each kit will be part of the file name for quick reference. This barcode can be found between the kit type and the date of service (when your results were released).

  • Clicking this purple link will open a new webpage where you can view your itemized receipt. One final link at the top of your receipt can be used to open a quick shortcut where you can print or save a copy of this document.
