How to use Request-A-Test to purchase an STI panel

Fellow partners with Request A Test to make it easy to get the STI tests you need at a LabCorp or Quest Diagnostics location near you.

An STI panel is not included with the Semen Analysis + Cryopreservation kit; however, our lab requires to have one on file to accompany your semen analysis.

We require the following test results to be on file. Fertility clinics may require different or additional tests, so please consult with your provider if you already have one.

  1. HIV 1 & 2

  2. Hep B (Surface Antigen)

  3. Hep C

  4. Syphilis

  5. HTLV 1 & 2

As a reminder, Fellow can only store samples intended for sexually-intimate partner use (SIP). We are unable to store samples intended to be used for donor or surrogacy use. This required STI panel reflects the requirements for SIP.


I. Order by phone:

Call Request A Test during business hours at 888-732-2348.

  • Mon-Thur: 8:00am-8:00pm EST

  • Fri: 8:00am-6:00pm EST

  • Sat: 9:00am-1:00pm EST

Tell the representative you would like to order β€œRequired Blood Testing for Fellow Cryopreservation”

If this is your first time ordering through Request A Test, the representative will request your

  • First name and last name

  • Date of birth

  • Address

  • Phone number

  • Email address

Clients who already have an account with Request A Test can just provide their name and date of birth.

You will be provided with a username and password for your Request A Test account. Please make note of this. You will need your username and password to access your test results.

The representative will help you select a lab location to have your blood drawn.

Before completing payment, provide the following discount code to save 10%: FELLOW2023

Payment for the order will be collected via credit or debit card (VISA, MC, DC, AMEX), prepaid VISA or AMEX gift card. HSA cards with a VISA, MC, DC, or AMEX logo are also accepted.

A receipt will be emailed to you immediately after payment is processed.

Once payment is received, your order will be processed. Order processing time is typically 3 business hours or less. Let the representative know if you need to get to the lab sooner. They may be able to expedite your processing time.

Add the following tests to your shopping cart:

New customers will register to set up an account or returning customers can log in.

Enter your zip code and select a lab location that is convenient.

Enter your payment information. Credit or debit card (VISA, MC, DC, AMEX), prepaid VISA or AMEX gift card. HSA cards with a VISA, MC, DC, or AMEX logo are also accepted.

A receipt will be emailed to you immediately after payment is processed.

Once payment is received, your order will be processed. Order processing time is typically 3 business hours or less.


III. Order Follow-Up

Once your order has been processed, you will receive an email with an order confirmation number.

When you go to the lab, take your order number and a valid photo ID.

Most lab locations accept walk-ins, however, to reduce your wait time you can schedule an appointment. This can be done at the website for the lab you have selected.

When your test results are complete, you will receive an email directing you to log into your account at www.requestatest.com to access your test results.
